What is capacity building support?

  We can support a person with a disability to participate in community life and have the same opportunities as other people.

 

What goals can capacity building support help me achieve?

 

Improved daily living

Develop the skills you need to get ready for the day or manage your life e.g. cleaning and cooking.

 

Increased social and community participation

Finding activities you enjoy and assisting you to join in.

 

Improved relationships

Includes social skills e.g. building confidence to have a conversation with a shop assistant or saying hello to others in the community.

 

What are therapy facilitators?

 

Therapy facilitators are workers trained to help you with your therapy plan.

They can work with you on a plan designed by your allied health professional who can be a:

  • Speech pathologist
  • Occupational therapist (OT)
  • Physiotherapist

 

How will therapy facilitators help me reach my goals?

 

Therapy facilitators can usually work with you one-on-one more often than an allied health professional.

This means you are more likely to achieve your goals!

 

Therapy facilitators have lots of special skills and training on how to follow programs designed by allied health professionals. 

We are a team working together to support you.

 

Where can I access CDS Outreach services?

   

We have workers living in:

  • Coonamble
  • Cobar
  • Coonabarabran

They can work with people that live around these areas.

What happens if I start using CDS Outreach, but its not right for me?

  You will sign a contract when you start working with CDS Outreach.
 

The service agreement says that there needs to be 1 months notice to end the agreement.

We can end the agreement sooner if there is an emergency.

  If you decide that CDS Outreach isn’t right for you, we will help you find something that is a better fit.
