About the Position

The Centre for Disability Studies (CDS) has a rewarding opportunity for a Practice Nurse with a passion for working with adults with intellectual disability. The position will be based in Sydney at our Camperdown office and specialist intellectual disability clinic in Strathfield.

ABOUT THE ROLE:

  • Key role in the provision of high-quality healthcare assessment, planning and coordination for adults with intellectual disability with complex health needs
  • The nurse will work closely with the clinic team, including the medical practitioner and allied health clinicians as required, as well as working independently to coordinate the service and complete health care plans
  • Coordination and management of referrals and the client clinical pathways
  • Providing timely and comprehensive written reports/health care plans either independently or in conjunction with the clinic team
  • Completion of reporting and evaluation activities of the service

ESSENTIAL CRITERIA:

  • Tertiary qualification in Nursing & minimum of 5 years full time post qualification experience
  • Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia (AHPRA)
  • NDIS Worker Screening Clearance (or willingness to obtain)
  • Knowledge of the NDIS and the disability sector, including NDIS Quality & Safeguards Framework
  • Comprehensive understanding of clinical governance in a disability setting
  • Experience in providing assessment and consultation for adults with intellectual disability and complex medical, physical and or mental health/ behavioural conditions
  • Demonstrate excellent interpersonal and communication skills with clinic staff, external agencies, people with intellectual disability and their families/carers
  • Excellent written communication skills, with a high level of attention to detail to produce succinct high quality reports
  • Demonstrated administrative and advanced MS Office Skills

ABOUT THE SERVICE:

The Centre for Disability Studies (CDS) is a not-for-profit research and education organisation that is affiliated with the University of Sydney. Our vision and mission is to create and disseminate knowledge that would improve the lives of people with intellectual disability. We achieve this through designing and developing research, teaching, and clinical practice to inform disability policy and practice.

The Complex Care Service will provide face to face and/or telehealth consultation for adults with intellectual disability with complex health needs. It will provide time limited comprehensive health assessments and health care planning, with the clients being referred back to their GP for ongoing care. The Complex Care Service will run a clinic one day per week (Thursdays).

The Complex Care Service is funded by the Ministry of Health via a Ministerial Approved Grant (MAG) until June 2024.
